THE HYBRID MODEL: Need-to-know info about daily requirements to certify your student’s health

Help us make sure your student is healthy by certifying your student's health each day their cohort attends school in person. 

You will receive an email and text message by 5 a.m. each morning your child attends school in person. It will remind you to submit a required online health certification form by the time your child arrives at school. Separate forms must be submitted for each child in your family. (BSD2 staff are already completing this form on a daily basis.)

DO NOT send your child to school if he or she is experiencing any of these symptoms. Instead, contact your child's school to report his or her absence.

  • Fever of 100.4 F or greater or chills
  • Cough
  • Shortness of breath or difficulty breathing
  • Fatigue
  • Muscle or body aches
  • Headache (not due to a chronic condition)
  • New loss of taste or smell
  • Sore throat
  • Congestion or runny nose (not due to known allergies)
  • Nausea or vomiting
  • Diarrhea
